How To Make An Appointment

By telephone: You may make an appointment by calling our appointment desk at 434-791-1088 during business hours from 9 am to 5 pm and following the instructions.

Online: You may request an appointment conveniently by filling out the following short online appointment form. Our appointment secretary will call you during our business hours to set date and time for your appointment.

Prior To Your Visit

In preparation for your visit, be sure to bring the following:

Social Security card
Insurance or Medicare cards
ALL your medication bottles (or a list of medicine names, dosages, and directions for all prescribed or over the counter medicines)
Previous medical records and any test results if available

Be sure to contact your referring physician 5 – 7 days prior to your appointment to ensure your medical information has been sent.

New patients should arrive 15 – 30 minutes before their scheduled appointment time to complete required initial paperwork. To save time, you also have the option to print a copy of the paperwork by clicking on the link below, and bring the already filled out and signed forms with you for your visit.
